Our user asked: "I am the cross country and track coach at my local high school. I have an athlete that is complaining of ankle pain. It started two days ago. He was warming up in gym class and the inside and back of his right ankle started to hurt. It does not hurt to squeeze his achillies. It seems to hurt near the bottom of the tibia just behind and above the notch on the inside of the ankle. This athlete says that it hurts when his foot is in a dorsiflex position. It has gotten worse and over the last two days and he did not run yesterday, and will likely not run today. Any ideas as to what this could be and/or what we can do to fix this problem."
 
Ask a PT Response: "I was wondering if your athlete injured his ankle in any way prior to the onset of these symptoms? Medial ankle injuries are alot less common than lateral ankle injuries and is usually associated when some abnormal force causes stress to this region. As far as what could be going on, there could be a number of injuries that could occur in that region such as medial ankle sprain of the deltoid ligament, fracture (ie. osteochondral, Pott's, etc.), tibialis posterior tendon injury, etc. My advice would be for him to see his doctor ASAP if he would like to be able to return to sport. Medical imaging of the ankle can be performed and will allow the MD to better understand what may be contributing to the pain."
 
Our user responded: "I had him take four days off.  It during the last two of those days he did not feel anything.  I found out that he has flat feet and he had been wearing nuetral shoes.  We bought him some motion control shoes.  He ran for 10 minutes yesterday and 25 minutes today and he was pain free.  I plan to have him do some cross training and see how he does.
 
Thank you so much for your reply.  I will let you know if there are further developments with his ankle."
