Our user asked: "Hi, I have a question. Should the patient sign his/her name elsewhere besides the sign in sheet such as on a daily note or billing sheet to state that they agree to services given to them? Thank you"
 
Ask a PT Response: "I've worked at quite a few clinics when I was a contract PT and all of the clinics including the one I am at currently have patients sign a financial obligation form at the initial visit which states they agree to PT services and are responsible for any costs which may not be covered by insurance. Having the patient sign again elsewhere is unnecessary."

And make sure it includes .... "Verification of benefits is not guaranteed". Many patients think that the verification is a guarantee and it can cause issues down the line.
