E-Stim settings for terminal knee extension/vmo

Our user asked: "what are the proper settings for electrical stim of the vmo to improve terminal knee extension of an elderly patient with right knee weakness and a left total hip. Appreciate your response"

Ask a PT Response: "Settings will vary depending on the type of machine you are using to facilitate quadricep strengthening. You should check with the recommended manufacturer's settings for that particular machine. At our clinic we utilize the Chatanooga Intelect Legend Combo and the Omnistim 500. For the Chatanooga unit we like to utilize Russian E-stim with mode- single, Cycle Time- 10/10, Frequency- 50bps, ramp- .5 sec and duty cycle- 50%. Intensity is determined by tolerance of the patient and the ability to generate a quadriceps contraction. For the Omnistim 500 we like to utilize the Medium Frequency Alternating Current (MFAC) at cycle time- 10/10, 80 hz burst/rate scan 20%, carrier frequency- 5.0, burst scan time- 8 sec and A/B simultaneous. Again intensity is determined by tolerance of the patient and the ability to generate a quadriceps contraction. No matter what machine you are using, the goal of using e-stim is to help assist in facilitating a muscle contraction. Therefore, check with your manufacturer's recommended setting for your machine.
