It's exacerbated with sit to stand from a very soft chair, accompanied by urinary incontinence. Nerve conduction was normal. Gluts were initially weak, stronger now with therapy. How old is the patient? Does the patient have a history of urinary incontinence and has cauda equina syndrome been ruled out? What resulted in the onset of pain?
